The welcome email you will get before your appointment will include a copy of our policies and fee schedule as well as a package of forms to complete.

For your first appointment, our provider will dedicate 30-50 minutes, during which he will gather information and discuss your personal goals for treatment. By the end of your initial appointment, a treatment plan will be formulated.

What To Expect

Our Policy

  • Scheduling

    All scheduling is done by phone or in person. You will receive an automated text message, phone call, or e-mail the day prior to your appointment.

    E-mail is not secure. We do not recommend using e-mail to address patient problems or inquiries. If you have a medical problem or question, please call us. Due to the high rate of no-shows for new patients, we require a non-refundable partial payment of $100, which would be applied towards the fees of the first new patient appointment.

    Follow-up appointments are 15 min, once a month unless the treatment plan discussed at the initial appointment requires more frequent follow-ups.

  • New Patients

    All new patients will receive our intake forms with our welcome email. It will include our policy, fee schedule, and a confirmation of appointment time and date. It will also include the forms that need to be completed BEFORE THE APPOINTMENT. Please complete the forms before the appointment. If that is not an option, please come in 30 min before the appointment time to get forms filled out in our office.

  • Administrative Fees

    – Professional Letter: $50

    – Form Completion: $100
    Disability, leave from work, physician statement, return to work (any form requested by patient or 3rd party).

    – Medical Records Request: $25 for first 20 pages and .50/page after that.

  • Cancellation Policy

    If you have to cancel or change your appointment, we require at least 24-hour notice before cancellation.

    No-shows or same-day cancellations for established patients are subject to a $60 penalty. New patients will not receive a refund for the $100 deposit.
